Kidney disease in cats is common, quiet, and far more manageable than the old name suggested. Chronic kidney disease (CKD) is what used to be called kidney failure — and in its early stages it is neither a crisis nor a countdown.

The early signs of kidney disease hide as virtues and habits: a cat drinking more, weeing more, a cat losing weight so slowly the scales barely seem to move.

Blood and urine tests can catch it years before it shows. Dilute urine is usually the first tell, SDMA rises early, creatinine late, and vets grade what they find as an IRIS stage from 1 to 4.

Pet parents often notice it at the bowl first: “she’s suddenly thirsty all the time.” Here’s why that observation matters so much.

Here is the reason, and it is the key to this whole article. Healthy cat kidneys are champion water-savers, concentrating urine to wring the most from every mouthful. As kidney tissue wears out, that concentrating ability is the first thing to go, so more water leaves in the urine, and the cat drinks more to keep up. The drinking is not the problem. It is the kidneys’ workaround, and it is often the earliest visible clue you will get.

What Chronic Kidney Disease Actually Is

Pet parents often hear “kidney failure” and brace for the worst. The modern name is kinder, and more accurate.

Chronic kidney disease means the kidneys’ filtering units are being lost gradually, over months to years. Lost units do not regrow (the disease is not curable), but the decline can often be slowed, its consequences managed, and good life protected for a long time. That is why vets retired “failure” for “disease”: most cats with CKD are living with it, not dying of it.

It is also very common. Overall, somewhere between one and three cats in every hundred have it; among cats past fifteen, roughly one in three, and some estimates run higher still. If you share your life with an older cat, this article is simply part of the territory.

The Early Signs of Kidney Disease Hide in Plain Sight

Pet parents often say, in perfect good faith: “she’s just getting old.” Here’s what ageing gets blamed for.

The classic early picture is a cat drinking more and weeing more: bigger clumps in the tray, a water bowl that empties faster than it used to. Alongside it, quieter changes: a cat losing weight gradually, often starting years before anything is diagnosed; a coat losing its lustre; appetite that dips and recovers; the occasional unexplained vomit. Each is easy to file under “old age”. Together they are a pattern, and the pattern has a blood test.

How Vets Catch It Years Early

Pet parents often type “cat kidney blood test” the night before an appointment. Here’s what the numbers actually mean.

The earliest signal is usually not in the blood at all. It is in the urine: a kidney losing its powers concentrates less, so urine turns dilute, and a dilute sample from a cat who should be concentrating rings the first bell, sometimes years ahead. This is the same thread that runs through our whole hydration cluster, seen from the other side.

Creatinine is the traditional blood marker, and it is honest but late: it often stays inside the normal range until roughly three-quarters of kidney function has gone. SDMA (symmetric dimethylarginine) is the newer marker, and it rises much earlier. Two honest footnotes belong beside it: specialist guidance does not treat SDMA as a stand-alone screening test, because non-kidney factors can nudge it. And much of its foundational research was conducted by the test’s developer with an industry partner, disclosed here because that is our habit, and the marker is now embedded in specialist staging regardless.

When disease is confirmed and stable, vets grade it on the International Renal Interest Society scale, as an IRIS stage from 1 (earliest) to 4 (most advanced). The grade rests on creatinine and SDMA read together, across at least two occasions, and is then sub-graded by blood pressure and by protein in the urine. The stage is not a verdict. It is a map reference — it tells your vet which treatments earn their place now, and what to watch for next.

What a Stage Actually Means

Pet parents often fear the number more than the disease: “stage 2 — is that bad?” Here’s the honest translation.

Staging measures where the kidneys are, not how fast they are travelling: some cats sit in an early stage for years. In published follow-up, cats diagnosed in the earlier stages typically survived for years, while the latest stage was measured in months; and survival studies count from diagnosis, so a cat caught early banks that time twice over. The practical reading: an early-stage diagnosis is not a countdown. It is the best possible moment to act.

The Treatment With Real Evidence Behind It: Food

Pet parents often ask, reasonably: “does the special food actually do anything?” This is one of the best-evidenced answers in cat medicine.

Yes — and the evidence is unusually strong. In a two-year randomised, double-masked trial in cats with moderate disease, not one cat eating the renal diet had a uraemic crisis and not one died of kidney causes. On the ordinary diet, roughly a quarter went into crisis and a fifth died of their kidneys.

An earlier clinic study found cats who accepted a renal diet lived a median of about twenty-one months against about nine, with the honest caveat that cats chose their group by what they would eat, so it was not randomised. Few interventions anywhere in veterinary medicine carry numbers like these.

The quiet hero inside the diet is phosphorus control. Worn kidneys struggle to excrete phosphorus, and the excess drives further damage; restricting it is thought to be the single most important lever the diet pulls. Where diet alone cannot hold the line, your vet can add phosphate binders to the food. Protein is moderated rather than slashed, and the diet carries other adjustments: this is why a proper renal diet outperforms any home approximation of one.

Getting a cat onto it takes the same patience as any food change: gradual mixing over a week or two, warmth, and zero ceremony. The Rest of the Playbook

Pet parents often ask: “what else can I actually do?” More than you might think — and less than the internet claims.

Water, everywhere, always. Hydration is protective and crises are usually dehydration-shaped, so this is where the whole cluster converges: wet or moistened food, stations in every quiet corner, fountains if she likes them, and never any restriction. In later stages your vet can teach you to give fluids under the skin at home: it sounds daunting and becomes routine, and cats tolerate it remarkably well.

Blood pressure. High blood pressure is a common companion to CKD and quietly damages eyes, heart, brain and the kidneys themselves. It is measurable in a calm consulting room and : talk to your vet about checking it at diagnosis, and on every recheck after.

The supporting cast, through your vet. Potassium supplementation when levels run low, anti-nausea and appetite support when eating falters, and treatment for anaemia in advanced disease. Each has its moment; your vet sequences them.

And the things to skip. No supplement, herbal blend or “kidney detox” has evidence anywhere near the diet’s, and some are actively harmful to a cat whose kidneys are already working hard. The honest hierarchy is unglamorous: food, water, blood pressure, weight — then everything else, by veterinary prescription only.

Living With It: The Rhythm That Works

Pet parents often wonder what the long game actually looks like. Mostly, it looks like an ordinary happy cat — with a calendar.
